How to use this form

Complete and return this form only if you wish to cancel an eligible contract. It is not obligatory: any clear statement that identifies you, the contract and your decision to cancel may be used.

You can use the electronic withdrawal function in your account. It lets you review the declaration before confirming it and sends an email acknowledgement with the date, time and protocol. You may instead copy the wording below and email it to pitchvintage@gmail.com.
